India is rapidly undergoing a demographic and epidemiological transition resulting in a growing proportion of older persons. Longer life span is accompanied by challenges of non-communicable diseases, physical decline, mental frailty, and cognitive deterioration. These varied conditions require both health and social care services in continuum with one another, though the two often operate in their own siloes. Our effort is geared towards planning integrated primary, secondary and tertiary healthcare services that are in turn integrated with social care services.
