This report highlights that the NP-NCD platform offers a comprehensive suite of six applications, supported by a platform designed for seamless service delivery from the grassroots level, such as ASHA workers, all the way up to tertiary care facilities. The platform ensures efficient management and monitoring through user-friendly dashboards, easily accessible downloadable reports, and administrative portals for relevant officers.

The primary goal of the NP-NCD digital platform is to create electronic health records and standardize the recording and reporting processes. It aims to assign a unique personal health ID (ABHA ID) to each individual, facilitating future health data sharing across service providers. The platform also streamlines referrals between health facilities, tracks follow-ups, assists in creating work plans for health personnel, and digitalizes health records for every person.
